What is the document for?
This document is needed for deregistration at the place of previous residence due to relocation. If the act of departure has been lost, the citizen must contact the Ministry of Internal Affairs at the new place of residence with a written application and detail all the circumstances.
The absence of a departure certificate is not an obstacle to filing an application for a new place of residence.
Russians are also deregistered:
- at the hotel;
- in the clinic;
- at a tourist base;
- in any place not intended for long-term residence.
If your stay was short, you do not need to declare deregistration. This only needs to be done if the citizen wants to leave earlier than planned.
When a person is discharged from the hospital, the departure certificate is issued by authorized hospital employees.

Suppose a citizen decides to personally apply for certificates in the established forms. Then he will have to:
- Prepare a package of documents. It will not be too large, but we will still get acquainted with its components further.
- Contact the authorized body with a request to issue a particular certificate.
- Receive a document of the established form within the specified time frame.
Everything is extremely simple and clear. Sometimes citizens verbally ask for forms 7 and 9. In practice, they may be asked to fill out an application of the established form. This is a very common practice. But even without a written request, you can still get certificates of registration and characteristics of housing.
Important: it is necessary to act in the same way if a citizen sends his representative to the authorized body.
What does a leaf look like?
The form of the document was regulated by Russian legislation. It must be filled in on both sides. The form contains a machine-readable data block that does not need to be filled out.
Citizens of the Republic of Belarus who wish to live and work in Russia need a Form 20 departure slip.
In Minsk and regional centers, this document is drawn up in 3 copies. In other settlements - in 2 copies.
After marking the departure, one copy is returned to the applicant along with an identification .
Departure acts
When changing his place of residence, a Russian can receive a certificate of departure at his previous address of residence. If such a document is lost, the person notifies in writing of the circumstances due to which the document cannot be presented. This is done at the newly chosen place of residence.
Important! However, the absence of an act of departure does not become an obstacle to the submission of application documents for the chosen place of residence and in subsequent procedures.
People become removed from the account:
- In a place of short-term stay in housing not intended for their long-term residence - at the end of the period of such stay.
- In hotels, hospitals, tourist centers and other accommodation organizations for guests - upon their departure.
There is no need to apply for de-registration at the place of short-term stay from an individual and to draw up an act of departure using form No. 7. An exception is if persons registered at the place of short-term stay intend to leave there before the end of the time specified by them earlier.
When discharging from medical institutions persons registered in them at the place of short-term stay, an authorized employee of this institution, within 3 days from the date of discharge, draws up departure certificates for them.
Number of copies - 2:
- The 1st document is sent to the local branch of the address bureau;
- 2nd - transferred to the regulatory authority.
If the FMS refuses to establish the status of forced migrants on the basis of messages from the FMS, at the end of 3 months from the date of registration of such persons at the place of stay in places of temporary settlement, the regulatory authorities remove them from the registration system in the manner established by the Administrative Regulations.
A mark on the removal of citizens from registration at the place of residence is placed on 2 copies of departure slips of form No. 7 (Appendix 7 to the Regulations), which are issued to citizens along with an identity card.

How to fill out the form
It is advisable to fill out the form on a computer. This is much faster and more convenient for both the applicant and authorized employees. But you can print the form and fill it out by hand.

There are 15 questions on the departure sheet. On side “A” there are points from 1 to 11. Answering them is mandatory.
On side “B” there are points 12 to 17. Point 12 is mandatory. Not everyone needs to answer the remaining questions.

Procedure for marking departure:
- Contact a department of the Ministry of Internal Affairs.
- Fill out form 7.
- After filling out the departure address sheet in Form 7, the authorized employee puts the date and certifies the document with a personal signature. This means that he has no comments on the information and documents presented.
- After completing the procedure, the Russian citizen is returned with a stamp.

Departure sheet: concept, purpose, nuances of filling out
The main purpose of the departure address sheet is to confirm the fact that the citizen no longer occupies his previous place of residence in the Russian Federation and has been deregistered there.

Situations when a sheet is necessary
The main situations when it is necessary to take care of obtaining a departure certificate include the following:
- the citizen leaves the place of temporary stay earlier than the period specified in the registration certificate. If he has issued a temporary registration in a hotel or sanatorium, then such a document is not needed when leaving home on time. But if for some reason you have to leave this place earlier, then the departure form must be filled out upon checkout;
- the migrant will be de-registered on his own and wants to register for a new place of residence as soon as possible. The fact is that if, during the process of registering at a new address, the registration authority does not have information about deregistration at the old one, a request will be made to the migration departments at the previous place of registration. In order not to waste time on such communication between government agencies, you can provide a sheet of departure from your previous place of registration, which will confirm that you are no longer tied to other addresses;
- It is planned to register foreign citizens for permanent residence in Russia. In this case, a certificate of departure from the migrant’s home country will be required.
Design features: information, rules, nuances
The departure slip is a standard approved form (Order No. 288 of the Federal Migration Service of Russia “On approval of the Administrative Regulations for the provision by the Federal Migration Service of state services for registration of citizens of the Russian Federation at the place of stay and at the place of residence within the Russian Federation” dated September 11, 2012).

The citizen’s personal data is entered into the columns on the first page of the document, namely:
- Full Name;
- Date and place of birth;
- citizenship;
- at what address it was registered (where it is leaving from);
- information about the registration authority;
- identity documents: passport, residence permit, birth certificate;
- place of new registration (where it arrives).
An arrival certificate is issued for the new place of registration (form 2).

Personal information on the back side is filled in depending on the situation. In this case, the following are required for everyone:
- clause 12 “Where did you come from”: the data from clause 8 of the front side is duplicated;
- clause 16 “The document was compiled”: the dates of the document’s preparation and the applicant’s signature are indicated;
- clause 17 “Deregistration formalized”: the date of deregistration is noted and the signature of the official responsible for the procedure is affixed.
If necessary, information is provided on the reverse side that the person is moving within the same locality or has previously changed his last name / other personal data (for example, after marriage).

The form must be filled out in two copies. - Fill out a new form in two copies.

A new sheet is issued to the applicant within a week. It is certified by the seal of the Ministry of Internal Affairs.
Contents of documents and correct completion
The document requesting to register the relationship has 8 points that must be filled out by persons wishing to obtain the status of spouses:
- Data on the full name of persons.
- Age information.
- Information about place of birth.
- Data on citizenship and nationality.
- Information about the place of residence of both persons.
- Details of the document confirming their identity.
- Information about previous registered relationships.
There are the following guidelines for filling out the form:
- To be completed by applicants themselves.
- Entering information must be done in ink that is either black or blue.
- The form should not contain various abbreviations.
- Data is entered only in accordance with documents that confirm the identity of the applicants.
- The date and signature must be placed only in the registry office in the presence of an employee.
- The age indicated is the one reached at the time of contacting the authority.
- Nationality is indicated at the discretion of individuals.
- If applicants were previously in an official relationship, then they must indicate the registration number of the termination of the relationship or the details of the document that confirms the death of the former spouse.
- Future newlyweds must enter information about the surnames they will have after the relationship is registered. Please note: the indication of the surname is carried out even when the spouses do not intend to change their premarital surnames.

Will they accept my documents for my new place of residence without Form 7?
Yes, they will. Previously, without this document, you could have been denied registration. But now the Regulations have been changed in your favor: documents are accepted, but the request is sent to your previous place of residence. The employee will provide you with a written explanation.

In this case, a duplicate will not be issued. You need to apply for a new passport.

Why do you need a departure slip and where to get it?
The departure address sheet (form 7) is included in the list of documents developed by the Main Directorate for Migration of the Ministry of Internal Affairs of Russia (formerly the Federal Migration Service) for the registration procedure at a person’s place of residence/stay in the Russian Federation.
