Who and why is taking form 7-injury in 2021
Using form 7-injuries, the state statistics service collects information on how many injuries and diseases occurred in Russian industries in 2021.
Legal entities of any form of ownership and types of activity are required to report on this form. But there are a number of companies that may not transmit information about employee injuries and illnesses to the statistics service. The following are excluded:
- micro-enterprises - less than 15 employees and 120 million in revenue;
- banks and insurance companies;
- organizations in the field of government or national defense;
- educational organizations;
- households;
- international companies and representatives of companies from other countries located in the Russian Federation.

Example of filling out a form
During 2021, Alpha and Omega LLC experienced industrial accidents with loss of ability to work for more than 1 working day: 2 workers and 2 workers were injured. At the same time, back in January 2021, cases ended with: temporary disability of an employee injured in November 2021 and the death of a foreign worker from an accident that occurred in December 2021.

Line 09 calculates man-days according to the production calendar (except for rest days) not worked due to illness.
Line 10 reflects the number of employees who worked without sick leave and were transferred to another position based on a medical certificate. Line 11 reflects the number of women included in the number of employees on line 10. Provided that sick leave was issued, there should be no information on lines 10 and 11.
Line 12 shows the number of persons who have the prof. disease.
Line 13 reflects the costs of labor protection measures in monetary terms, which are recorded according to accounting data (clause 4 of the Instructions).
In this case, the “For reference” section must be completed:
14 – Average number of employees – reflects all employees, both full-time and external part-time workers, except for persons working under a civil partnership agreement
15 – Number of women in the average number from line 14, except for women working under a GPC contract and on maternity leave
16 – Availability of a health center – marked Yes or No
The form must be signed by the official who is responsible for submitting the form and with the obligatory reflection of: his position, telephone number, email, date of execution of the document.
Legal basis of form No. 7-injury, changes 2021
The form is designated as 7-injury, and as management documentation it is assigned the code 0609304. The Statistics Service collects information on how many injuries and illnesses occurred in Russian industries in 2021. The procedure for filling out the report and the current form were approved by Rosstat Order No. 417 dated June 21, 2017.
Since the beginning of 2021, the worker injury and illness report has not changed at all. However, it is worth remembering that along with the main form 7-injury, you must take the application every 3 years. It contains data on accidents that have occurred by type and reveals their causes. The frequency of delivery is fixed at the level of law; it is not related to the operating time of the business entity. In 2020, Rosstat expects a main report with an appendix.
Innovations for employers
Starting with reporting for 2021, Rosstat is introducing an annex to form 7-injuries. For the first time, it must be submitted along with the report itself no later than January 27, 2020, indicating information for the current year. The application form was approved by Rosstat order No. 493 dated August 10, 2018. It also contains instructions for filling out the document. It is important to understand: the Form 7 injury report is annual, and the supplement to it is submitted only once every 3 years (for 2021, 2022, etc.). It is permitted not to prepare additional information if there were no accidents at work during the reporting year and there is nothing to report.

Filling out form 7 injuries: report structure and detailed instructions
Form 7-injury consists of a title page, a large table of indicators and a reference part. Let's look at each component in more detail and give step-by-step instructions.
Title page
The title page should include information that is standard for all statistical reports. To begin, indicate the year for which you are submitting the report - 2021.
Enter the name of the organization in the appropriate line, as it is given in the registration documents, and indicate the short name in brackets.
In the “mailing address” line, enter the name of the subject of the Russian Federation, city, street and house, do not forget to indicate the postal code that corresponds to the legal address. If you actually work in another location, then indicate the current address where you operate.
Tabular part
Information about work-related injuries and injured employees should be taken from reports in Form N-1, which are drawn up for each accident. Data on the organization of security measures and the costs of them can be taken from accounting.
The entire table contains 13 rows. For each line, you must indicate the requested indicator for 2021 in the units of measurement specified in the third column. Let's look at the lines with which difficulties most often arise.
Line 01 is the result line. It must indicate the number of employees injured at work if the accidents resulted in the inability to work for 1 working day or more or resulted in death.
This line does not need to take into account deaths in the reporting year that occurred as a result of accidents that occurred in the years preceding the reporting year. The same applies to employees whose temporary disability began in 2021 and ended in 2019.
In lines 02 - 04, it is necessary to distribute the number of people from lines 01 into categories: women, children and foreigners.
On line 05, indicate the number of deaths due to industrial accidents during 2021. The time of occurrence of the accident does not matter if the death occurred in the reporting year. In lines 06 - 08, also categorize them as women, minors and foreign citizens.
On line 09, enter how many days during the year all employees remained unable to work. To do this, you need to sum up the number of all days of incapacity for work as a result of accidents at the organization's production site. Take information from medical reports for employees whose disability ended in 2019. If it is still ongoing, then it can be ignored.
In line 10, highlight the number of victims who partially lost their ability to work and, in accordance with the medical report, were transferred to easier work for a period of 1 working day. In line 11, highlight the women. These lines do not need to take into account employees who received sick leave.
In line 12, enter the number of employees who were diagnosed with a disease related to their profession for the first time in 2019. An employee can be included in the line on the basis of a medical report from a specialized organization and an internal document (directive, order).
If labor protection measures were carried out during the year, then indicate the amount of expenses for them in line 13. The source of the funds does not matter.
Help part
In the reference section, indicate the three requested indicators. On line 14 - the average number of employees, on line 15 - the average number of female employees. These include full-time employees and external part-time workers. Women on maternity leave are not counted.
In line 16 it is necessary to put o if the company has a first aid station or doctors who are able to provide first aid to victims.
Common mistakes when drawing up Form 7
The presence of errors and inaccuracies is observed when the instructions telling how to fill out individual lines of the form are not carefully studied:
- Instead of person-days of incapacity for work, the total number of days of illness is indicated.
- An accident that occurred last year is being re- noted and information has already been submitted about it.
- The lines are left blank, and their filling is mandatory. For example, the average number, the amount spent on labor protection.
To reduce the number of possible errors, the Instructions for filling out Form 7 contain recommendations regarding logical control. They should be studied carefully and test formulas should be used.
Form and place of submission of report 7-injuries
If the company has separate divisions, then for each of them you need to fill out and submit a separate form. The additional form is submitted for a legal entity without taking into account its divisions.
It is necessary to report to the territorial body of the FSGS, which corresponds to the legal address of the legal entity or its division. In cases where the place of registration and place of business do not coincide, the form is submitted to the authorities at the place of business.
The form can traditionally be submitted on paper or electronically.
Where to submit the report
The report must be submitted to the territorial body of Rosstat at the location of the separate division (for this OP) and at the location of the legal entity (without separate divisions). If an organization (its separate division) does not operate at its location, then the form is provided at the place of actual activity.
If the company and separate divisions are located in the same subject of the Russian Federation, then the report is presented as a whole for the legal entity, including data on the “separate divisions”.
If a legal entity has separate divisions located on the territory of different constituent entities of the Russian Federation, data is submitted for each OP at their location. In this case, it is allowed to provide a summary report for all divisions operating in a specific subject of the Russian Federation. In this case, the responsibility for submitting the report is assigned to one of the departments.
If the organization did not conduct any activity during the reporting period, then it must also submit the form by filling out lines 23-25.
When a legal entity is reorganized in the form of transformation, the successor submits reports (including data on the predecessor) for the period from the beginning of the reporting year in which the reorganization took place.
Bankrupt organizations that have entered bankruptcy proceedings must also provide information in the specified form.

Responsibility for failure to submit a report
Organizations that failed to submit a report on time or included false information in it will be subject to economic sanctions under Article 13.19 of the Administrative Code. There are fines for these violations.

Employer's liability
For violation of the deadline for submitting statistical reporting and for reflecting incorrect information in it, the head of the enterprise bears administrative responsibility.
If a statistical report is not submitted to Rosstat or false data is indicated in it, the company faces a fine under Part 1 of Art. 13.19 Code of Administrative Offenses of the Russian Federation. You will have to pay:
- from 20,000 to 70,000 rubles. — the organization itself;
- from 10,000 to 20,000 rub. - to its officials.
A new violation will cost more:
- for officials - from 30,000 to 50,000 rubles;
- for legal entities - from 100,000 to 150,000 rubles.
